Thank you for choosing the FBG Clinic as your primary care provider. To become a new patient, please fill out the forms below or stop by our office to pick up new patient information packet. After you have filled out and returned your new patient packet, along with a copy of your insurance card, we will start the insurance verification process and send your new patient packet to your choice of provider for approval.  You should expect to be contacted by your provider of choice within 3 weeks from the time you submitted your new patient packet. However, if you need to be seen for an acute visit, please notify our staff when returning your new patient packet,  and we will get you scheduled to see one of our Nurse Practitioners as soon as possible.
